Recognition triggers
These are patterns that may warrant evaluation — not a diagnosis checklist. The clinical determination is yours.
Persistent swelling
The limb stays enlarged despite appropriate compression, exercise and elevation.
Heaviness and function
Heaviness, tightness, aching, reduced range of motion or limited walking distance.
Skin changes
Hyperpigmentation, hyperkeratosis, fibrosis, lymphorrhea or skin breakdown documented by the clinician.
Recurrent cellulitis
A history of recurrent infection or other skin problems in the chronic edema population.
Wounds
Wounds or impaired healing where edema control is limiting progress.
Garment tolerance
Difficulty tolerating or consistently using appropriate compression, or shoes and clothing no longer fitting.
Not first-line therapy
For Medicare lymphedema, a home pneumatic compression device is not first-line treatment. Conservative therapy comes first, and coverage can follow when the treating practitioner determines that significant symptoms remain after the required trial.
